FAMILY CARE OF NEW JERSEY'S MAIN GOAL IS TO PROVIDE HIGH-QUALITY HOME CARE SERVICES TO OUR CLIENTS. WE UNDERSTAND THAT NO INDIVIDUAL IS THE SAME! HENCE, OUR AGENTS ARE READY AND WILLING TO WORK WITH CLIENTS DIRECTLY TO DEVELOP A UNIQUE CARE PLAN FOR EVERY INDIVIDUAL. WE PROVIDE TWO MAIN SERVICES: CARE MANAGEMENT AND PRIVATE HOME HEALTH CARE.

CARE MANAGEMENT

Initial Consultation - Our care manager will conduct an intake and gather information about the client. During this process, our care manager will explore the strengths and limitations of the client and assess the client’s overall physical and mental well-being. 

Care Plan Development - After the intake, we start working on developing a unique care plan that utilizes the client’s strengths and addresses the client’s presenting issues. In the care plan, we design interventions and service recommendations that will improve the client’s quality of life and enrich the environment.

Care Coordination - We monitor the care that the client is receiving and evaluate the efficiency of the services. We serve as client advocates and help clients navigate the complex healthcare system. We ensure that the client is receiving the appropriate level of care from all participants involved in his/her care. 

Supporting Client Independence by Providing In-Home Accessibility Modifications - We provide referrals to interior designers trained in helping older adults stay independent in their homes while also ensuring their safety. We also partner with organizations that can carry out the recommendations put forth by the interior designer. Every in-home modification varies depending on the client’s needs.

24/7 Support - We are here to help 24 hours, 7 days a week. If the client is experiencing a crisis, we will provide timely and sensitive crisis interventions. 

Daily Check-ins - We monitor the care of our clients by checking in with all the participants in the client’s care. 

Caregiver Support - Our caregivers have 24/7 access to care managers. Our private companions/home health aides keep daily reports and communicate everything to our social workers. Our care managers provide training to our caregivers on a constant basis.

Pulled Trust Application - Sometimes, our clients will need to resort to other resources to receive the level of care they need. We can fill out the pulled trust application for you and apply for any other additional resources to guarantee that the client is receiving the best care possible. 

Referrals to Community Resources - We provide referrals to our trusted resources such as physical therapists, occupational therapists, nutritionists, medical specialists, and other relevant professionals to complement the care that the client is receiving.

Financial Planning - We provide financial planning services to our clients who would like to know what would be the most affordable and efficient service delivery within the budget that they have. 

Bio-Psycho-Social Assessment - We complete a bio-psycho-social assessment with our clients and use it to evaluate the care plan that is already in place. Bio-psycho-social assessments help both the care manager and client to understand the care plan and augment the services the client is receiving. 

LGBT+ Affirming Care - Our care managers are trained to provide affirming and culturally-sensitive care to LGBT+ identifying older adults. Our interventions are tailored in a way that considers the unique life experiences of LGBT+ older adults. 

PRIVATE HOME HEALTH CARE

PERSONAL CARE

Aides and companions at Family Care of New Jersey perform the following personal care activities:

  • Bathing

  • Assisting in tub

  • Total care in shower

  • Assisting in turning in bed

MOUTH/DENTURE CARE, HYGIENE

Maintaining client’s hygiene is one of the most important roles of our home health aides. We make sure to make our client’s life easier and better. Our companions carry out the following activities:

  • Hair care - comb, shampoo

  • Grooming - assisting with nails and shaving

  • Skin care

  • Foot care

  • Toileting - diaper, commode, bedpan/urinal, toilet assisting

NUTRITION

Our home health care aids are trained to create a unique diet for each individual based on their needs. HHA specialists can determine the structure and portion of each meal of the day for our clients. Aids will be cooking three times a day - breakfast, lunch and dinner. (An additional cooking meal will be provided if necessary, depending on the patient or client). Family Care of New Jersey agents curate the following:

  • Diet/food allocation

  • Cooking - breakfast, lunch, dinner

  • Assist with feeding

  • Recording intake - food/fluids

ACTIVITY

Our home health aides are trained and certified to perform the following procedures:

  • Assist a client with bed transferring

  • Assist with walking

  • Show and help a client how to use a cane, walker or crutches

  • Our employees can design special home exercise programs for clients
    Exercises include - assisting clients with a range of motions such as exercising R arm, L arm, R foot, L foot and neck

TREATMENT/SPECIAL NEEDS

Our Aides/Companions are capable of performing various health related treatments and measurements such as:

  • Take oral, rectal or axillary temperature

  • Check pulse

  • Take respiration

  • Check blood pressure

  • Weight patient

  • Record output (Urine/BM)

  • Assist with catheter care

  • Empty foley bag

  • Assist with ostomy care

  • Remind to take medication

  • Assist with specific treatments as directed by the nurse or a doctor

PATIENT SUPPORT ACTIVITIES

At Family Care of New Jersey, we love to think that we provide more than just home health care aides. We train our employees in various activities and believe that they need to exceed the expectations of our clients. Our aides and companions can perform the following patient support activities:

  • Change bed linen

  • Do patient laundry

  • Perform basic housekeeping duties such as cleaning the kitchen, bathroom, client’s room, and patient’s care equipment

  • Our aides can do shopping for clients

  • Do patient errands

  • Accompany patient to medical appointment

  • Help clients with reading a book

  • Monitor client’s safety
